What Is Sofwave? A Dermatologist's Review

Vancouver dermatologist Dr. Katie Beleznay explains how Sofwave works, what results to expect, downtime, and who may be a good candidate.

Sofwave has become one of themore talked-about non-surgical skin tightening treatments. Here is how I explain Sofwave to patients.

What Is Sofwave?

Sofwave is a non-invasive ultrasound treatment designed to improve skin firmness and mild to moderate laxity. It delivers ultrasound energy at a depth of approximately 1.5 mm, within the dermis, where controlled heating triggers a wound-healing response and stimulates collagen remodeling.

Unlike a laser, Sofwave does not resurface or remove the outer layer of skin. That is one reason downtime is generally limited.

How Does Sofwave Work?

During treatment, the handpiece delivers ultrasound energy into the dermis while integrated cooling helps protect the skin's surface.

The goal is not to create an immediate dramatic lift. The treatment stimulates a gradual remodeling process, so changes typically develop over the following weeks and months as collagen is produced and reorganized.

A treatment generally takesabout 30 to 60 minutes, depending on the areas being treated.

Who Is a Good Candidate for Sofwave?

I tend to consider Sofwave for patients who:

- Are starting to notice mild to moderate laxity, particularly around the lower face, jawline, neck, or brow

- Want a non-surgical treatment with little downtime

- Prefer a gradual, natural-looking change

- Have realistic expectations about what a non-surgical tightening device can achieve

The last point matters. Sofwave is not a facelift replacement. If someone has significant skin laxity and wantsa substantial lift, an energy-based device may not be the best use of their time or money.

What Results Can You Expect?

For the right patient, Sofwave can produce a subtle improvement in skin firmness and laxity. Results are gradual, with improvement commonly assessed around the 12-week mark and sometimes continuing beyond that.

Some patients are happy after one treatment. Others may choose additional or maintenance treatments depending on their degree of laxity, response, and goals.

Results are not generally dramatic. The appeal is that it can help the skin look a little firmer without changing the shape of the face or requiring surgical recovery.

How Long Do Results Last?

Sofwave stimulates your own collagen remodeling, but it does not stop the aging process. How long someone notices a benefit varies with age, skin quality, sun exposure, and other individual factors.

Some patients choose maintenance around the one-year mark, while others wait longer or do not feel they need another treatment.

Is Sofwave Painful? What Is Recovery Like?

Sofwave has minimal downtime, the treatment is not sensation-free. Patients can feel short bursts of heat or discomfort as energy is delivered, but how people tolerate the treatment is variable so comfort measures are often used.

Afterward, mild redness,swelling, or tenderness can occur, but most people can return to their usual activities quickly.

Sofwave vs. Ultherapy or Thermage

These treatments are sometimes grouped together, but they are not interchangeable.

Sofwave uses ultrasound energy at approximately 1.5 mm in the mid-dermis. Ultherapy also uses ultrasound but can treat at multiple depths, including deeper tissue planes. Thermage uses radiofrequency rather than ultrasound.

I do not think one device isautomatically “best.” The more useful question is which technology makes the most sense for your anatomy, degree of laxity, treatment goals, and tolerancefor discomfort and downtime.

The Bottom Line

Sofwave can be a good option for mild to moderate skin laxity when the goal is gradual tightening with minimal downtime.

For me, good results start with choosing the right patient and setting realistic expectations before treatment.
